Confidential Client Information

Consultant’s Commitment:

• I agree to give you my undivided attention and professional assistance during your scheduled consultations, and I agree to strict confidentiality.
• I am professionally committed to using my training and skills to assist you in mobilizing your own resources to achieve your own unique goal(s).
• I agree to use the best of my abilities to help you make the changes you seek and to treat you with care and respect.
• I may draw upon methods such as hypnotherapy, Neuro Linguistics, life coaching, and other alternative healing methods, all of which are uniquely called upon for your benefit.

Client’s Commitment:

• Your success depends on your own commitment to improving the situation which you wish to work on.
• This session is not meant to diagnose, prescribe, or treat any physical or mental ailments, and the consultant is not responsible for or treating any medical condition you may have.
• You release the consultant from any liability whatsoever regarding your hypnotherapy/life coaching session. Any conclusions you draw from your session or responses you have are yours and yours alone. You take full and total responsibility.
• You understand that these sessions are not psychotherapy but are a therapeutic alternative aimed at creating positive changes in your life.
• You agree to be on time for appointments, understanding that if I you are late, it will shorten the time of your session.
• Appointments must be cancelled or rescheduled at least 48 hours in advance.
• You agree to inform the consultant of all physical or mental conditions which might affect the work. (Example of physical, mental, or emotional conditions which may apply: epilepsy, cancer, AIDS, clinical depression, phobias, panic attacks, psychosis, schizophrenia, anorexia, bulimia, alcoholism, drug dependency, etc.)
